Glen Oaks Community College is committed to maintaining an environment where students, employees, and guests can learn, work, and engage in campus life safely and respectfully. Reporting concerns helps the College respond quickly, provide support, and protect the well-being of our community.

If you observe behavior or experience a situation that may impact health, safety, or campus conduct, please use the appropriate reporting option below.

If the situation is an emergency or involves an immediate risk of harm to yourself or others, contact 911 before submitting any form. These reporting forms are not monitored after hours, on weekends, or during college holidays.


How to Report a Concern

Review the categories below and select the form that best fits your situation. Each form is routed to the appropriate College office for review and follow-up.

If you would prefer to speak with someone confidentially before submitting a report, visit the Student Services Office and ask to speak with a licensed counselor.

What kind of report would you like to file?


Academic Integrity Violation

Use this form to report academic integrity concerns such as cheating, plagiarism, falsification of information, or other forms of academic dishonesty. This process helps protect the value of a Glen Oaks education and ensures fairness for all students.


Student Conduct or Concerning Behavior

Students, faculty, and staff are encouraged to report behaviors that may be concerning, disruptive, or threatening, even if the behavior seems minor.

Examples may include classroom misconduct, aggressive or hostile behavior, threats or acts of violence, significant changes in behavior, disorganized speech, confusion, or substance misuse. Early reporting allows the College to provide support and intervene when needed.


General Complaint or Request for Assistance

If you have a concern or issue related to the College that you have been unable to resolve, this form allows you to request assistance.

Examples may include concerns about customer service, challenges with a policy or procedure, or complaints involving a faculty or staff member.

Whenever possible, students are encouraged to first speak directly with the individual involved, as many concerns can often be resolved informally. If that is not possible or comfortable, submitted concerns will be reviewed and directed to the appropriate Vice President or administrator.


Sex Discrimination, Harassment, and Retaliation (Title IX)

The College strongly encourages reporting incidents of sex discrimination, sexual harassment, sexual violence, dating or domestic violence, stalking, or retaliation.

Reports are reviewed by the Title IX Coordinator, who can provide information about rights, reporting options, and available resources.

Students who wish to speak confidentially without filing a report may meet with a licensed counselor in Student Services, a private counselor, or a member of the clergy.


Student Housing Concern

Residents and guests of the Devier Student Suites may use this form to report housing-related concerns or potential violations of the Housing Handbook.

This includes issues involving residents, guests, or community standards within the residence hall. Facility or maintenance requests should be reported directly to the residence hall front desk staff.
